Airline Ticket Give-A-Way

Prize: Win Round Trip Airfare for two (you and a companion) to any United/Continental Airlines destination in the Continental United States and the Caribbean. Tickets subject to all United/Continental rules & restrictions
Rules:
- This is a Give-a-way for Attendees of the Institute of Healthcare Improvement’s 23rd National Conference in Orlando, FL in December 2011.No purchase is necessary to enter the give-a-way
- Attendees must visit the Hospital TeamCare booth to pick up a ticket to enter the give-a-way
- The winning ticket is selected by random drawing of the winning ticket from a glass bowl containing all of the Attendee’s tickets that participated in the give-a-way
- The winning ticket number will be posted on the www.TeamCare.com website, Monday December 12, 2011.
- The winning ticket will be posted on the website for one week, and if not claimed, additional tickets will be drawn until a winner is identified.
Winner must:
- Contact Hospital TeamCare Headquarters by phone or email within one week
- Send the winning ticket (which must match the paired winning ticket retained by Hospital TeamCare within 2 weeks of winning ticket announcement.
- Provide Hospital TeamCare necessary information which airline and TSA requires for issuance of airline tickets (name, dob, address), etc
